Holding ctrl-alt-shift while you click on the photoshop shortcut to start it up will make it ask you if you'd like to reset the prefs.

I have a Sidewinder mouse (5-button) which comes with a tool for setting program-specific commands on the extra buttons. So I have two buttons under my thumb for zooming in and out.
