How do you know when to use the spot healing brush without the content aware option selected or with it selected?
Unfortunately I have found that the only way to know what works best for a certain situation is good old trial and error.
As an example, I have attached a torn photo. Without trial and error, can you tell if you are going to heal the torn area with the content aware fill selected or not?

Forget content aware for that problem. It's going to be a case of clone and maybe channels to restore.
