The clone tool in Photoshop should do the trick for what you want.

To remove something, you have to consider what is supposed to go behind it. If you have a similar place in the image you can clone it there. However if you don't have a similar plaec, you have to draw/paint it in.

So now you have the hand away by painting or cloning over it. Now you have to check the remaining part of that limb. Has it just happened? You need blood and some fleshparts hanging about.. some gore. If it's an amputation or something, then the end has some scare-tissue here, paint them in. Afterall a limb is 3 dimensional and it needs a decent ending to make it seem real.
