Opacity stuck in Photoshop CS5

The opacity is stuck in CS5. It'll say the layer , flow and brush are at 100% opacity, yet every stroke is transparent. Last time it happened, I closed PS then reopened it and it started working. Not anymore. So if anyone can help that'd be great.

Could be a painting mode of the brush or a blend mode of the layer causing the stroke to appear transparent.

If this is not the cause, taking a screenshot of Photoshop with the problem present on screen can help myself and the other users here track down what the problem is.
