This photograph is of one of cousin's wedding and I loved this effect! I was wondering what was used to white out the background while still having that nice color on the ground.

I'd guess they just used a soft eraser to remove the background to expose a solid white layer beneath it.
Nice concept, but I'd think it could be done better using a gradient.

It almost looks like they used an envelope (judging by the 45 degree angles), stock photo, either already with a distressed look and/or additional customised grunge bushes.

I would a use a soft brush in combination with a (not too strong) cloud brush (to keep the fog effect intact). The use of the cloud brush will make the effect look even more impressive if done well (don't overdo it).
