I had a pic given to me to fix this week. I can't post it here for copyright issues, but the lady in it had lovely yellow teeth, so I though I'd post a tip on fixing it.
With the lasso tool, draw carefully around all the teeth. Now select/feather 2 pixels (just to soften the edge).
Now the clever part: image/adjustment/hue saturation - in the drop down box select the yellow option and just take down the saturation, this leaves the teeth looking natural but not over white as they can if you use levels and set white point.
You can use this to remove red eye, as well, but instead of yellow, choose red.

Hi There-
Here is one that I use for the same issue and I find it works great.
1 After opening the photo, select a new layer.
2 Select the Dodge tool
3 In the tool bar, make sure you select midtones.
4 Start the exposure at 50%
5 Maginify the photo so you can see a large amount of teeth (for detail) You don't want to whiten the gums. (LOL)
6 Pick a small brush to get around the teeth without hitting the gums.
7 Now move the Dodge tool around the teeth and you will see them whiten right up. If it is whitening to fast, drop the percentage and it will whiten at a slower speed.
8 Make sure you zoom out from time to time and make sure that it looks O.K. and not "to white" A zommed in look can be misleading.
9 Thats it, it is pretty simple once you get the hang of it. Oh, It also works great for the eyes as well.

I find that using quick mask and then desaturation leaves teeth looking a wee bit grey if you have to take much color out.
I zoom out to nearer normal scale and carefully move the lightness up and down (or do it with brightness/contrast) until the teeth look natural.
