Holding shift down to paint a straight line no longer works

I seem to lose, regain and then lose the ability to shift+paint/erase in straight lines with photoshop. If i hold shift and click at one point with my brush, then click at another point, surely photoshop should fill the distance between the two points? Is there a setting i'm missing or have deactivated, perhaps?

Make your first click (without holding the shift key) then hold the shift key and make your second click.

If you want a perfectly horizontal or vertical line..hold click and drag in the side or up direction.
