the basics:
1. make a pattern
2. fill a new layer with that pattern,
3. adjust fill opacity
4. add drop shadow.
5. use quickmask and gradients to select the parts you want to fade out.
5 press delete.
please tell me which things you dont know how to do and ill show you. i dont want to explain patterns and gradients and layers if all you need to know is how to make it fade. if you want ill show you everythign though. with screenshots

ok. sorry, i didnt reply for a while i was a bit busy. its not that complicated, it looks long because of all the screenshots as you request. also you said you were a beggeiner and i explained alot of things, most tutorials assume you know. its really not difficult.
1. ok start out with a pattern of what shape you want to make. you need to make this shape the size of squares you want to show. decide how big you want your pattern. id make it smaller if your not sure because you can usually size up easily, but no size down (esp with the mesh one.) this is important, if you are making a small banner, you would want to use a very small pattern.
2. open a new file with those demensions, and checkmark transparent, for this example i am using a high resolution picture, and am using 30x30 pixels.
3. for mesh one, make something that looks like this:
the thickness of that red stripe will be the thickness of the mesh
for the squares, something like this:
the thickness of the blank/clear area, is the distance between each square.
it doesnt matter which color you use now, i am using red to show better, its easier to see. you can change the color later to anything. also i have zoomed in to show better, the image is 30x30, very small.
4. go to edit>>define pattern, and save it.
5. open up the image you want to apply the effect to.
6. create a new layer. go to Shift+Ctrl+N, or use the drop down layers menu, Layers>>new>>Layer
7. Make sure that layer is selected
and go Edit>>>fill. chose Pattern, chose your custom pattern.
should now look like this
8. if you want to change the color, go to your layers pallete and hold ctrl and click on layer one. this feature selects everything in that layer.

9. fill with desired color, ctrl+backspace for background color, alt+backspace for foreground color. you can also use the Edit>>fill menu. ctrl+D to deselect.
i am changing to white.
10. in your layers pallete, double click layer 1. you will see a blending options screen. here are some effects you can use, i reccomend, drop shadow, inner shadow, or bevel/emboss. play around with these and their settings.
11. for now, I just checkmark drop shadow and leave its default settings.
12. near the top of the layr pallete there are two options, opacity and fill. these both refer to opacity. Opacity refers to the whole the layer as a whole, blending options effects and all. the second refers to the opacity of only the fill color of the layer, and not the effects added in teh last step. once again, play around. I am putting fill at 15% and opacity at 75%
13. now for the fading. make a laker mask by clicking the layer mask button(see above) make sure you are working on layer 1. you acan also go Layers>>add layer mask>> reveal all.
layers pallete should now look like this:
with a layer mask, whatever is white will show, ans whatever is black will not show.
14. make sure you have selected the mask, and not the layer itself. using the gradient tool, fill what you want to be clear as black.
15. you can also use a fuzzy eraser brush instead of the gradient tool. for simple things this can be very easy. just erase what you want gone. it will show up as black on teh layer mask. if you mess up, use the paintbrush and paint it back. this will show up white. you can change the size of the brush with [ and ] buttons.
AND YOUR DONE!!!

when i was learing pshop, it frustrated me no end to read in a tutorial that told me how to do it, but didnt explain it and i would have a hard time applying tools i just used to other aplications.
yeah its long but it covers layer masks, gradients, patterns, the layer pallete, blending options and opacity.
