Looking for a DVD Case template in photoshop, HELP !!!!

Hi All-

I was wondering if anybody can tell my how to make a DVD Template in photoshop. What I'm looking for is just a template that I can do the graphics for the front and back and the spine of a DVD case. The measurements of the front and back are 5" across and 7.25" tall, the spine in the middle is 9/16" wide by 7.25" tall.

Any help you can give would be great. I can create a textured and colored background and create some text effects and stuff like that to create my home movie casees. I just don't know how to get a blank templete set up in the same file to get started with.
