How was this logo/text created?

How was the logo below created?
I'm trying to re-create this logo myself but I have not been successful.
I've tried created a text layer and then above created a gradient layer and "masked it" (if that's the right word) to the text layer so the text have that gradient effect. Then I tried adjusting transparency but I can't get it to look even close to what its supposed too.

When viewing this it does NOT have a white background. This is a very transparent logo so please download it to view it properly. (.PNG)

Doesn't look like anything more then a gradient overlay and an inner shadow on the top to me.

The image part would be the same but on a shape (shape tools / pen tool)

I hate to be a pest but if possible could you try re-creating the image and then upload the photoshop file on dropbox or gmail or some other file sharing site for me to download?

I've tried to do this but it's just not working out very well.
