Problem erasing masks completely

Whenever I create a layer mask and erase by painting black a small amount of the layer always remains and is visible. Looks like something at a very low opacity.

This happens even after I tried filling with black, and using a brush and grad at 100% to erase. Regardless of what I do a slight residue of layer still remains.

Anyone know what's going on?

Something somewhere is not at 100%. Or, something is selected. I'd make sure the black brush is solid.

That's the only thing/things that can be doing this.

I found the problem .. when I create a mask the default white & black colors show but it turns out my black was 95% gray, so it never took away all the area even when filling.

So now At least I know what's causing the problem and for now I simply click on the white / black icon to restore the true 100% black.

I still don't know why the default color is 95% gray though.
