Generally, I'd recommend that you pay for hosting and your own domain name, rather than using a free solution. The reason for that is because it's important to control both your name and your data.
You touched on the URL part of it - buying your own domain name allows you to have your URL and, more importantly, the ability to take that URL wherever you want. Sometimes, with a free host, when you leave a service, you leave your URL, which can be a major blow.
The same for data, too. It's not acceptable to start over content wise. That's why it's so important to have control of your data.
To answer your questions:
1. Usually "forever," but forever is always tied to the existence of the service, of course.
2. Yes. A web host can shut your site down, as well, if you're doing questionable things.
3. Some of them do. Often times, you can request features, as well.
4. Some do, some don't. Some allow you to use your own domain name for free or for a charge.
