CS3 Cursors not working

I'm running CS3 Photoshop on an Intel Mac, OS X.4.10

I set the cursors in preferences to:
Painting : Normal Brush Tip
and
Other Cursors: Precise

However when I use the Brush tool, healing tool, clone, etc. I only get the "Precise" crosshair (the cursor that looks like a plus). This really sucks because I can't see how large the brush is nor the area I'm painting. Holding down shift key doesn't help either. I reinstalled PS and the problem persists. Only when the brush tool is below 40 pixels does the Brush cursor that I want (the brush circle) show up. Please help me force Photoshop into using the brush cursor ... thank you.

Make sure your Caps Lock is off
