Usually the hand appears when you have the spacebar held down. It helps you navigate the image easier. As mentioned above I'd try tapping the spacebar again when in photoshop.

Just untick the "Contiguous" button and it will select all the white areas.

Hi, I'm retouching some old family photo's and could do with a bit if help.

I'm able get rid of most of the scars and boost the colours of the old image as below:-
