Let us know if you don't have Illustrator - there are ways to do this purely in Photoshop too, though not as easily.

If I understand what you're asking, you need to go to

Image -> Canvas Size

Then input "14 pixels" for the width and height of the image.
Then, use the little selection boxes to pic ...

To me, this screams native 3-D software. Photoshop was probably used to composite the fake faces with the people, but it seems almost certain that the face/masks themselves were rendered elsewhere.
